Algone Premier New Patients
To ensure your first appointment is successful, please fill out the new patient paperwork online or arrive 1 hour prior to your appointment time. We try our best to stay on schedule, so it is VERY IMPORTANT that you choose Option 1 or 2 and follow through with the instructions.
Options 1
Fill out the new patient paperwork completely online and arrive 30 minutes before your appointment with the physician. By arriving 30 minutes before, it allows time for the provider and staff to look over your paperwork and ensure its accuracy and analyze your history.
Options 2
Arrive 1 hour before your physician appointment and fill out the paperwork in our office. Late patients or patients who arrive at the APPOINTMENT time without completed PAPERWORK, will not be seen, and must be rescheduled for a later date.
